Transitions Co-ordinator

There are two transitions co-ordinators in each Board area.  A Transitions Co-ordinator is available to provide advice and support and information to pupils aged 14+ who have a statement of Special Educational Needs and their parents / carers in preparation for adult life for potential post-school options.

 

The role of the Transitions Co-ordinator is:

 

  • to ensure a person-centred approach is in place by working in collaboration with the pupil, parents, school, careers service, health and social care trusts professionals and health transition workers, voluntary and other agencies;

  • to ensure the provision of comprehensive and coordinated information about the range of options available;

  • to ensure that all necessary follow-up actions are carried out within the context of the Code of Practice on the Identification and Assessment of Special Educational Needs and the recent  (September 2005) Supplement to the Code of Practice, and Article 4 of the Education (NI) Order 1996;

  • to produce a coherent transition plan at the first (and subsequent) annual review of the statement of special educational needs after the pupils 14th birthday and to share the transition plan between all professionals involved.
