Parent / Carer Contribution

A Parent / Carer can help by ensuring that:

 

  • They play an active part and valued role in the young person’s education.

  • Some time has been spent with the young person finding out what their hopes and wishes for the future might be.

  • Encouragement is given to the young person to be fully involved in decisions about their life.

  • They express their views and those of their child from an early stage in order to ensure that the Plan for Transition is flexible enough to meet their needs.

 

Parents / Carer may find it useful to make a checklist of everyone who has been working with the young person and who has information that will be important in the review meeting. Not everyone who is invited will be able to attend the meeting therefore written information from professionals who know the young person is important in devising the Transition Plan.
