Your child will have a Transition Plan meeting at the age of 14 in order to plan their transition process over their last years in school.
This meeting aims to put in place all the necessary measures to ensure a successful transition, identifying and overcoming barriers or difficulties in the process.
The Transition Plan will be reviewed along with the statement at each annual review meeting within their last years of school.
There will be various people that may attend the transition plan meeting:
- The Young Person
- Parents / Carers
- Principal / Teacher
- Careers Advisor
- Education Transitions Co-ordinator
- Representative from your Health and Social Care Trust
- Other Voluntary Organisations
The transition plan will start to identify the young person's:
strengths
support needs
goals
aspirations
Issues of independence, work, skills development and health will also be considered.
During the review parents / carers and professionals will decide on tasks that will assist the young person and their parents / carers with the transition to adult life. The Transition Plan will identify those individuals with responsibility to carry out these tasks.
Arrangements for the co-ordination, monitoring and reviewing the transition plan will be the responsibility of the the school supported by the Transition Co-ordinator together with the Specialist Careers Adviser and any other appropriate professionals.
The transition plan is not a static document but a document that recognises that an individual's needs may alter over time.
